Thank you for taking the time in reading this application. I am a single mother of two boys, and have worked my way from being homeless and in a transitional living program for 11 years. Finally I was able to purchase my own home 3 years ago. This has meant everything to me.
Last summer I was off work for 3 months due to a shoulder surgery. Recovery time took longer then expected and I fell behind in all my bills; my gas connection has even been cut off, and I'm working my hardest to get it back on. To this day I am still playing catch up. My budget is very tight right now and I have a medical bill from the surgery that keeps haunting me.

I am having a very difficult time and would appreciate any help you can offer me. Having this bill paid would make me forever grateful.
